Considering the ending of the insurance contract for the supplementary treatment insurance of the Chemical and Polymer Industry Union of Pardis with Iran Insurance Company, Pars Rahiab Insurance Company, Code 3050, dated 2018/05/23, please attend to complete the requested documents in the table from 2018/05/18 within 10 working days In the union, from 9 am to 14 pm, to register and submit requested documents as well as settling financial balance.

It should be noted that this date will not be extended.
